Title of Position Available: Bi-lingual Medical Case Manager (Spanish)

Department: Client Services

Position reports to: Medical Case Management Program Supervisor

Date to be filled: Immediately

Brief description of the responsibilities: Provide medical case management services and education to Spanish speaking individuals with HIV/AIDS.

Minimum educational/experience/degree: Bachelor’s (required) or Master’s degree (preferred) in Social Work or a Bachelor’s or Master’s in a human service related field with experience working with target population.
