Case Management

Upon comprehensive assessment, clients are referred to either Medical or Non-Medical Case Management as determined by the client’s level of need.  Those clients who demonstrate a relatively low level of need are referred to a Non-Medical Case Manager.  A client who presents with multiple, ongoing needs and requires coordination and follow-up of medical treatment, adherence counseling, and service linkage is referred to a Medical Case Manager to ensure access to an appropriate level of support.

A case manager serves as a liaison between the client and community resources to ensure that support systems for people living with HIV/AIDS are available.

In addition, a case manager can also meet the client where he/she is most comfortable (home, hospital, doctor’s office, etc….) Generally, initial assessments are conducted in the office. For those clients that are referred to Medical Case Management, a home visit is usually also conducted.

Requirements:
  • Be HIV+
